  • RN Group Home Manager  –  Amherst

    RN GROUP HOME COORDINATOR

     

    OBJECTIVE OF POSITION: This position works closely with the Group Home Coordinator to provide the individuals residing in Wall Residences group homes quality health care that meet and/or exceeds best practices, Licensure, Medicaid regulations and Wall Residences policies and philosophies in a manner that is consistent person-centered practices.  The RN must practice under the supervision of a Registered Nurse or physician and in accordance with applicable laws and other practice guidelines.

     

    DUTIES OF POSITION:

    • Promotion of Wall Residences philosophy, mission, and values within and outside of the agency by way of networking, creating connections, and leading efforts toward creating and supporting quality health care services for our clientele.
    • Serve as a role model and mentor to staff by demonstrating sound team-based leadership, creative problem-solving skills and a collaborative work environment.
    • Work with nursing staff to provide medical oversight and quality assurance at the group homes.
    • Complete and submit all required documents in a timely manner.
    • Model good use of organizational resources. This includes car pooling, combining trips, prevention of waste, etc.
    • Maintain working order of computer and other technology. Problems with technology should be reported to the Group Home Manager or office personnel.
    • Attend administrative and staff meetings associated with the Group Home.
    • Serve as the direct supervisor of staff.
    • Serve as a role model and mentor to staff concerning professionalism, respect and team building.
    • Provide training to Group Home staff as required to maintain best practices and quality services.
    • Conduct unannounced visits to the medical group home on all shifts to evaluate all operations and staff performance.
    • Conduct monthly documentation reviews of Group Home staff and provide direction as necessary.
    • Work with other nursing staff to assess all individuals and assure that staff can safely be taught the skills to meet each individual’s care needs and providing training and oversight to this end.
    • Attend transition and referral meetings for potential individuals entering our services.  Use these meetings as an opportunity to assess for skilled nursing needs for nursing care plan development.
    • Maintain an active and unencumbered license to practice as a Registered Nurse in Virginia.  A copy of this current license must be on file at Wall Residences central office.
    • Provide and document all nursing care using the approved forms and documentation systems.
    • Provide direct care as necessary and in accordance with the practitioner’s scope of practice and level of experience.
    • Develop Nursing Care Plans for the individuals we serve and update as warranted.
    • Provide emergency interventions until emergency responders arrive based on trainings and level of experience.
    • Provide administrative oversight of staff that includes Nurses and Directed Support Professionals.
    • Manages overall operation of group home.  Including staff, scheduling and other administrative functions.  There are on-call responsibilities with this position.
    • Delegate care and responsibilities to other staff as necessary to ensure quality operations and management of group homes.
    • Ensure that an individual’s residential support plan(s) and nursing care plans are carried out as written in a professional, supportive and person-centered manner.
    • Immediately report unusual conditions, incidents or emergencies to appropriate authorities and to the supervisor according to the Wall Residences Policy, Human Rights Plan, and the individualized instructions in the client’s service plan.

     

    Wall Residences employees are mandated reported of abuse and neglect.  Suspected abuse must be reported to the Group Home Coordinator, Regional Wall Residences Staff/Directors as well as the Department of Social Services Adult Protective Services worker, as appropriate, immediately, no later than 24 hours after observation.

    Wall Residences is currently seeking a Full-Time experienced Registered Nurse Care Manager in the Lynchburg and surrounding counties area.

     

    OBJECTIVE OF POSITION: To provide the individuals residing in Wall Residences’ Sponsored Residential services and/or Group Home quality health care via nursing consultation, training and nurse delegation that meet and/or exceeds best practices, Licensure, Medicaid regulations and Wall Residences' policies and philosophies in a manner that is consistent person-centered practices. The RN must practice according to the scope and standards of practice of the Registered Nurse outlined in the Nurse Practice Act and other applicable laws and practice guidelines.

  • Group Home Manager  –  Lynchburg Area

    GROUP HOME MANAGER

     

    OBJECTIVE OF POSITION: Create a positive home environment for the individuals who live in the assigned homes and the employees who work there. Homes should be operated in licensing and Medicaid compliance while using person-centered thinking practices.

     

    DUTIES OF POSITION:

    • In coordination with other agency personnel, hire, train, schedule and supervise Direct Support Professionals who work in area group homes.
    • Oversee the scheduling of staff to determine that schedule requires minimum overtime. Staff scheduling may be completed by Team Leaders.
    • Conduct regular staff meetings and provide notifications to staff of agency updates by email.
    • Provide weekly e-mail updates to management of all activities with extra e-mail and telephone communication as significant events occur.
    • Promotion of Wall Residences philosophy, mission, and values within and outside of the agency by way of networking, creating connections, and leading efforts toward creating and supporting quality services for our group home residents.
    • Complete and submit all required documents in a timely manner, specifically employment paperwork for new employees, and invoices for services delivered.
    • Manage decisions about purchases within agreed upon authority limits and with good communication with accounting and supervisory staff.  Purchases are to reflect prudent use of agency resources.
    • Maintain working order of computer and other technology. Problems with technology should be reported to management or office personnel.
    • Participate in service plan development in collaboration with Program Managers and Case Managers for each individual in the homes and oversee the implementation of plans by all staff. Plans for Support should reflect in a meaningful way, what really works for the individual.
    • Development of a staffing schedule that meets the needs of the individuals served in the group homes and that reflects staffing levels consistent with agency direction and budget limitations.
    • Ensure homes remain operational by ensuring necessary supplies are available such as groceries, cleaning supplies, etc.
    • Assure that the homes are well maintained and in good working order both inside and out and for vehicles and equipment.  Ongoing maintenance contracts will be in place for most functions.  It is the job of this position to coordinate and schedule all routine maintenance.  Emergency and unscheduled maintenance is to be reported by GH staff to this person and other on-call personnel.  A timely response to all emergency needs is expected along with the reporting of all actions to administration.
    • Attend transition and referral meetings for potential individuals entering the group home. Use these meetings as an opportunity to assess needs for inclusion in service planning.
    • Assess all individuals and assure that other staff can safely be taught to meet each individual's care needs and provide training and oversight to this end.
    • Ensures that all documentation in the service meets licensing, legal and best practice requirements.
    • Accept and check-in all medications (for homes that do not have nursing services) from the pharmacy verifying they were filled correctly and match physician orders. Also, verify that Medication Administration Records match physician orders and pharmacy medication labels.
    • Maintain good communication with staff who provide separate activities programming for the individuals residing in the assigned group homes. The Group Home Coordinator should communicate the need for changes in the activities programming to the WRAP Coordinator.

     

    Wall Residences employees are mandated reporters of abuse and neglect.  Suspected abuse must be reported to the Group Home Coordinator, Regional Wall Residences Staff/Directors as well as the Department of Social Services Adult Protective Services worker, as appropriate, immediately, no later than 24 hours after observation.
