  • Staff Nurse  -  Danville

    STAFF NURSE

Wall Residences is hiring a Part-Time Staff Nurse (RN) in the Danville, Virginia Area that can provide quality healthcare to individuals residing in Wall Residences group homes.

     

    Qualified candidate must be able to provide professional nursing care in accordance to best and current medical practices, maintain confidentiality, promote person-centered care, communicate effectively and have good computer skills. Experience in Skilled Nursing and/or Home Healthcare preferred. Must be team oriented, a problem-solver and have excellent organizational, time management, documentation skills, and possess the ability to work independently.  The RN supervises the LPN and other staff within the group home.  The Registered Nurse reports to the Group Home Manager and agency Directors.  The RN is responsible for the health and wellness of the individuals in their care and of all who are served by the agency.

     

    Knowledge, Skills and Abilities expected of Employee

     

    • Maintain an active and unencumbered license to practice as a Registered Nurse in Virginia.  A copy of this current license must be on file at Wall Residences central office.
    • Provide and document nursing assessment using the approved assessment forms.
    • Assess all individuals and assure that other staff can safely be taught to meet each individual’s care needs.
    • Write and assess Nursing Care Plans for individuals who require ongoing health monitoring and direct nursing oversight.
    • Implements the prescribed ordered and delegates duties in accordance with the Nurse Practice Act.
    • Educate and monitor staff providing care on an ongoing basis.
    • Interface with hospital professionals to assure proper discharge of individuals back into the service.
    • Immediately notify the physician of any changes in the condition of an individual.
    • Provide education, instruction and delegation as needed to other staff in accordance with the guidelines provided by the Registered Nurse.
    • Serve as a role model and mentor to staff concerning medical and care tasks assigned.
    • Provides clinical and supervisory oversight to the LPNs and other service staff.

  • Nurse Manager (RN)  -  Roanoke/Hillsville

    Wall Residences is currently seeking an experienced Part Time RN Nurse Manager to provide the individuals residing in Wall Residences sponsored residential, and group homes quality health care that meet and/or exceeds best practices, Licensure, Medicaid regulations and Wall Residences policies and philosophies in a manner that is consistent person-centered practices. The RN Nurse Manager must practice per the scope and standards of practice of the Registered nurse outlined in the Nurse Practice Act and other applicable laws and practice guidelines. The RN Nurse Manager is responsible for providing nursing consultation, oversight and training in regards to the individuals on their caseload. This position will include Roanoke and New River Valley areas. An agency vehicle will be provided.

     

    DUTIES OF POSITION:

     

    ·   Promotion of Wall Residences philosophy, mission, and values within and outside of the agency by way of networking, creating connections, and leading efforts toward creating and supporting quality health care services for our clientele.

    ·   Create and maintain a case load, and skilled nursing activity that provides billing that covers the cost of the Nurse Manager salary and other program expenses.

    OBJECTIVE OF POSITION: This position works closely with the Group Home Coordinator to provide the individuals residing in Wall Residences group homes quality health care that meet and/or exceeds best practices, Licensure, Medicaid regulations and Wall Residences policies and philosophies in a manner that is consistent person-centered practices.  The RN must practice under the supervision of a Registered Nurse or physician and in accordance with applicable laws and other practice guidelines.

     

    DUTIES OF POSITION:

    • Delegate care and responsibilities to other staff as necessary to ensure quality operations and management of group homes.
    • Ensure that an individual’s residential support plan(s) and nursing care plans are carried out as written in a professional, supportive and person-centered manner.
    • Immediately report unusual conditions, incidents or emergencies to appropriate authorities and to the supervisor according to the Wall Residences Policy, Human Rights Plan, and the individualized instructions in the client’s service plan.

    Wall Residences is currently seeking part time experienced Direct Support Professionals (DSPs) to provide direct support services to individuals with intellectual disabilities, other developmental disabilities and/or mental health issues as outlined in the Individual Service Plan / Plan for Support. To participate in training and community activities per directions and scheduling provided by the worker’s supervisor and described in the Individual Service Plan.

     

    Being available and ready to work as scheduled is of primary importance in this position. The worker is required to locate another worker to work for them in the event of a need to be absent.
