  • Operations Quality Assurance Coordinator - Richmond or Lynchburg

    POSITION DESCRIPTION:

    The function of the Operations, Quality Assurance Coordinator is to assure that agency policy and procedures are current and that agency personnel are properly trained and supported to implement these operational systems. The Operations QA Coordinator is responsible for optimizing utilization of technology systems with consultation from IT contractors and other personnel. Technology, policy and training are continually monitored and problem solved to improve and assure application of best practice in the field in addition to quality and consistent operations for the agency as a whole. The overall goal of this position is to analyze, evaluate, develop and supervise systems within each area of services to ensure efficiency and that maintain agency compliance with regulations, to support all personnel to consistently perform essential duties, and make efficient use of all available resources.

     

    The Operations Quality Assurance Coordinator is part of the Quality Assurance team which is tasked with working in unison to assure the overall health of the agency as it relates to policy, planning, clinical, medical, and operational functions. As a member of this team, it is the responsibility of the Operations Quality Assurance Coordinator to be part of team decision making for overall agency functioning.

     

    QUALIFICATIONS

    Substantial knowledge and experience with the state ID / DD regulatory environment and with applying Wall Residences policy and procedures. Must have excellent leadership and instructional skills for others in the field to include knowledge of technology systems. A bachelor's degree in the Human Services field is required.

     

    DUTIES OF POSITION:

    Operations

    • Understand trends and proposed system changes and the potential impact on the agency and services provided. Report on potential outcome with the QA team. Engage in discussion with state, regional, and local entities regarding system and service impact.
    • Ongoing review of Licensing, Medicaid, and Human Rights and other pertinent regulatory requirements. Communication of any changes or clarification of regulatory requirements within Wall Residences. Training of agency staff.
    • Implement required policies and procedural changes across services and departments to develop tools for operation and management. Provide guidance and training, on updated or existing policy and procedure manuals. Assure systems, documentation, protocols and policies meet all requirements.
    • Ensure that all systems can be maintained on the Electronic Healthcare Record.
    • Assess the need for and develop training for employees and contracted workers in the areas of systems, operations, and protocols. This includes providing support and clarification regarding Wall Residences, Human Rights, Licensing, and Medicaid policies and well as person centered thinking and planning.
    • Be aware of day to day operations with regard to referrals, potential providers, vacancies, case load information, employment issues, provider strengths and needs, complaints, etc.
    • Routine review and updating of forms, templates, processes, protocols, as needed and based on regulatory changes, suggestions or agreement of best practices
    • Streamline overall functionality of tools and technologies available. Includes but not limited to: f-drive/electronic files/Electronic Health Records, data base, web site, SharePoint, and WaMS.
    • Supervise and Coordinate with the Compliance Technology personnel around reporting systems and agency wide compliance with HIPAA, human rights, Licensure, Medicaid and other regulatory systems compliance.
    • Act as resource for responses to investigations, CAPs, or HR violations and recommendations. This includes providing oversight and guidance for responding to citations or recommendations made by Licensing and/or Human Rights.
    • Routine review of records and documentation via electronic review, on-site review and/or faxed documentation from the direct care provider to assure Medicaid, Licensing and agency requirements and standards of quality. Periodic or as requested record reviews to assist with evaluation of personnel performance.
    • Provide guidance and direction to Program Managers and Coordinators on SIS quality reviews and serve as a liaison to the Regional Support Specialists and leaders overseeing the SIS assessment process to promote approvals of necessary SIS re-assessments.
    • Provide written reports of findings of audits, reviews, site visits, and the like to the QA team for review and follow up.

    Wall Residences is currently seeking part time experienced Direct Support Professionals (DSPs) to provide direct support services to individuals with intellectual disabilities, other developmental disabilities and/or mental health issues as outlined in the Individual Service Plan / Plan for Support. To participate in training and community activities per directions and scheduling provided by the worker’s supervisor and described in the Individual Service Plan. This is a full-time day shift position.

     

    Being available and ready to work as scheduled is of primary importance in this position.  The worker is required to locate another worker to work for them in the event of a need to be absent.

    • Directly provide support services to individuals in their home, in a center, or in community settings.  This includes completion of training for short-term objectives and assistance activities listed in the client’s service plan.

    Wall Residences, Inc. is currently seeking a Client Files Specialist in our busy Floyd, VA office.

     

    The Client Files Specialist position includes several roles that assist in all operations of Wall Residences, Inc. These roles include, but are not limited to: records clerk, information gathering, and data entry. The most important element of all work performed is accuracy. Knowledge of documentation, data collection and record keeping systems, regulations regarding confidentiality and licensure requirements and Community Intellectual Disability services is a plus.

     

    Candidate must also have a high level of proficiency with Microsoft Office Suite, Excel, and Data Entry. Position is full-time, office hours 8:30 am to 5:00 pm Monday-Friday.   • RN Group Home Coordinator  -  Lynchburg area

    OBJECTIVE OF POSITION: This position works closely with the Group Home Coordinator to provide the individuals residing in Wall Residences group homes quality health care that meet and/or exceeds best practices, Licensure, Medicaid regulations and Wall Residences policies and philosophies in a manner that is consistent person-centered practices.  The RN must practice under the supervision of a Registered Nurse or physician and in accordance with applicable laws and other practice guidelines.

     

    DUTIES OF POSITION:

    • Promotion of Wall Residences philosophy, mission, and values within and outside of the agency by way of networking, creating connections, and leading efforts toward creating and supporting quality health care services for our clientele.
    • Serve as a role model and mentor to staff by demonstrating sound team-based leadership, creative problem-solving skills and a collaborative work environment.
    • Work with nursing staff to provide medical oversight and quality assurance at the group homes.
    • Complete and submit all required documents in a timely manner.
    • Model good use of organizational resources. This includes car pooling, combining trips, prevention of waste, etc.
    • Maintain working order of computer and other technology. Problems with technology should be reported to the Group Home Manager or office personnel.
    • Attend administrative and staff meetings associated with the Group Home.
    • Serve as the direct supervisor of staff.
    • Serve as a role model and mentor to staff concerning professionalism, respect and team building.
    • Provide training to Group Home staff as required to maintain best practices and quality services.
    • Conduct unannounced visits to the medical group home on all shifts to evaluate all operations and staff performance.
    • Conduct monthly documentation reviews of Group Home staff and provide direction as necessary.
    • Work with other nursing staff to assess all individuals and assure that staff can safely be taught the skills to meet each individual’s care needs and providing training and oversight to this end.
    • Attend transition and referral meetings for potential individuals entering our services.  Use these meetings as an opportunity to assess for skilled nursing needs for nursing care plan development.
    • Maintain an active and unencumbered license to practice as a Registered Nurse in Virginia.  A copy of this current license must be on file at Wall Residences central office.
    • Provide and document all nursing care using the approved forms and documentation systems.
    • Provide direct care as necessary and in accordance with the practitioner’s scope of practice and level of experience.
    • Develop Nursing Care Plans for the individuals we serve and update as warranted.
    • Provide emergency interventions until emergency responders arrive based on trainings and level of experience.
    • Provide administrative oversight of staff that includes Nurses and Directed Support Professionals.
    • Manages overall operation of group home.  Including staff, scheduling and other administrative functions.  There are on-call responsibilities with this position.
    • Delegate care and responsibilities to other staff as necessary to ensure quality operations and management of group homes.
    • Ensure that an individual’s residential support plan(s) and nursing care plans are carried out as written in a professional, supportive and person-centered manner.
    • Immediately report unusual conditions, incidents or emergencies to appropriate authorities and to the supervisor according to the Wall Residences Policy, Human Rights Plan, and the individualized instructions in the client’s service plan.

     

    Wall Residences employees are mandated reported of abuse and neglect.  Suspected abuse must be reported to the Group Home Coordinator, Regional Wall Residences Staff/Directors as well as the Department of Social Services Adult Protective Services worker, as appropriate, immediately, no later than 24 hours after observation.

    Wall Residences is currently seeking an experienced Direct Support Professional (DSP) in Culpeper, Virginia, to provide direct support services to assist individuals with disabilities and health concerns with activities of daily living and community integration in their home as outlined in the Individual Service Plan / Plan for Support. To participate in training and community activities per directions and scheduling provided by the worker’s supervisor and described in the Individual Service Plan.

     

    PART TIME POSITION. MUST HAVE WEEKEND AND EVENING AVAILABILITY.

     

    DUTIES OF POSITION:

    • Directly provide support services to individuals in their home or in community settings. This includes completion of training for short-term objectives and assistance activities listed in the client’s service plan.
    • Complete documentation for program implementation, medication administration and to report on activities and unusual conditions or events per Wall Residences policy and procedure and State and Federal regulations.
    • Participate in training activities and organizational meetings as required.
    • Other work duties as needed or assigned.

     

    Knowledge of:

    • Principles and philosophy of developmental disabilities and intellectual disabilities services and mental health issues.
    • Concepts of positive behavioral supports
    • Specialized and general knowledge of the implementation of individualized training programs
    • Documentation and data collection systems
    • Licensure regulations including human rights policy and other standards and laws
    • First Aid, CPR, infection control and medication administration
    • Client Rights/Human Rights Policy
    • Mandated reporter requirements
    • Emergency response procedures

     

    Skilled at:

    • Oral and written communication
    • Interpersonal skills particularly in terms of building trust and rapport with persons who have special needs.
    • Use of “Person-First” language
    • Observing and documenting behavior
    • Responding to the special needs of people with disabilities
    • Implementation of training programs for special needs populations

     

    Ability to:

    • Develop and maintain positive relationships with individuals who experience disabilities.
    • Maintain appropriate professional boundaries
    • Use common sense approaches to implement services
    • Accurately document activity in a timely manner
    • Carry out training and behavior support programs
    • Appropriately implement professional confidentiality practices
    • Demonstrate a commitment to quality, community based services for people with disabilities
    • Communicate to others the nature and philosophy of the services offered.

A bachelor’s degree in human services with at least one year of experience in the field of developmental disabilities (DD) is required along with a driver's license, good driving record and excellent references.
