Sponsored Residential homes (sometimes called family caregivers) match our highly qualified Providers with supported individuals to live in the Provider’s home. This model of care offers a highly personalized small setting, with each Provider supporting one or two people. Our signature matching process emphasizes fit and many individuals live with the same Provider for decades.

Community Engagement (CE) & Community Coaching (CC) supports adults with developmental disabilities to aquire, retain, or improve skills necessary to navigate safely through their local community and form natural support relationships. DSPs (Direct Support Professionals) guide individuals in small groups (CE) or one-on-one (CC) through activities designed to give opportunities for skill building.

Wall Star Centers are group day support programs that provide a safe space for adults with developmental or intellectual disabilities to socialize with peers, explore new interests, volunteer in the community, and enjoy activities. Participants also reinforce skills such as cooking, money management, and behavior avoidance.

Wall Community Options’ In-Home Care Services provide support to individuals living in their own home, either by themselves or with their natural family. The goal of this program is to allow the person to live as independently as possible with a maximum quality of life within their home and community.

Serving Individuals with Disabilities in Wall Services and Programs

Regional RN Care Managers are assigned throughout Virginia serving the regions with RN consultation, medical policy/procedure development, health/medical quality assurance, health protocol development, training, and more. The RNCM provides advocacy for optimum care and medical interventions which may be person-specific or to improve systems of care.

Serving Individuals with Disabilities in Wall Sponsored Residential or Group Homes

This service of nursing is individual-specific and may take place daily to monthly based on the person’s needs. It is supported by a nursing care plan which is individualized and person-centered, and a part of the Plan for Support. This service is specific to an individual’s clinical and functional needs. This service is less than 25 hours per week.

Serving Individuals with Disabilities in Wall Sponsored Residential or Group Homes

This service is used minimally to support individuals with intensive care needs and is supported with a nursing care plan which addresses the care tasks essential to a person’s health and safety. This level of nursing also requires a medical provider order for Medicaid approval and is supervised by the RN Care Manager for the individual’s region. This service is up to 40 hours per week.
